Instructions on filling
out your petition form
Each form has a few
sections to fill out. Some sections you will fill out yourself as the
circulator. Some sections the voters will fill out, and others will not
be filled out at all.
(The actual
form is not color-coded.)
It is YOUR
RESPONSIBILITY to fill out the green
sections.
COUNTY
Before
gathering signatures, you must enter the name of the county in which
the signers are registered to vote. This means that all of the
addresses on the form must be from that county. NO EXCEPTIONS.
You are
the circulator. It is your responsibility that every signature
on the sheet:
Was witnessed by you, and Is from a registered Oregon voter.
You must
sign the sheet before you send it back to Ben, or it will be entirely
disqualified. DO NOT CONTINUE TO COLLECT SIGNATURES AFTER YOU HAVE
SIGNED AND DATED THE FORM.
The
voters fill out lines 1 to 10. Make sure they fill out each field
fully -- signature, date, name, and address. Make sure that their
name is printed legibly. Each voter may only use one line and each
line may have only one voter on it.
